CORE Pathway
The Coordinated Orthopaedic Recovery & Excellence (CORE) Pathway is a specialized fracture liaison service focused on improving identification of patients in the ED, standardizing inpatient care, improving patient outcomes, and ensuring comprehensive follow-up care. We design and implement evidence-based clinical pathways tailored to your organization's needs, with a specialized focus on hip fracture management. Our pathways support geriatric-focused care initiatives, establishing multidisciplinary care protocols, providing a framework for high-quality fracture management, and building the infrastructure needed for receiving ACEP Geriatric Emergency Department accreditation.
After the Fall
Our customized patient education booklet, After the Fall, provides patients with everything they need to know about recovering from a fragility fracture. This material teaches patients what to expect following their injury/surgery, how to optimize healing and improve bone health, and how to create a fall-proof living environment to help reduce to the risk of future falls.
Provider Education & Training
We deliver specialized education and training programs for Physician Assistants and Nurse Practitioners looking to expand their clinical skills in fragility fracture management. Through 1-on-1 mentorship, our evidence-based curriculum ensures your providers are equipped to lead a CORE Pathway on their own.
